Awwww....we are sorry to see you go! However, if you wish to terminate your membership, please fill out the below fields. Once this form is submitted, this will serve as your 30-day written notice to stop your next bank draft. You will receive a confirmation email from our Membership & Operations Director, Mike Coolidge stating he has received your request. If you do not receive this email confirmation in 24 hours (Monday-Friday) please contact Mike directly at 262.248.6211 or firstname.lastname@example.org to confirm your termination.
- If you decide to rejoin within three months from the date of termination, we will waive the joiner fee. After three months, the joiner fee will apply.
- Membership fees are not refundable.
- If you still want to be a member, but want to put your membership on hold you can do this! A draft/EFT membership can be put on hold for a maximum of three months, one time per calendar year. Just click here to fill out a Membership Change Form. Simply print out that form, complete and return to the YMCA in person which is required for any "on hold" request.
