Membership Change Form

Awwww....we are sorry to see you go! However, if you wish to terminate or make a change to your membership, please fill out the below fields. Once this form is submitted, this will serve as your 30-day written notice to stop your next bank draft or make necessary adjustments. You will receive a confirmation email from our Membership & Operations Director, Mike Coolidge stating he has received your request. If you do not receive this email confirmation in 24 hours (Monday-Friday) please contact Mike directly at 262.248.6211 or to confirm your change. 

Please note:

  • If you terminate membership and decide to rejoin within three months we will waive the joiner fee. After three months, the joiner fee will apply.
  • Membership fees are not refundable.
  • If you still want to be a member, but want to put your membership on hold you can do this! A draft/EFT membership can be put on hold for a maximum of three months, one time per calendar year.
