Membership Change Form

If you wish to cancel your membership, or make a change to your membership, please fill out the fields below. Once this form is submitted, this will serve as your 10-day written notice to stop your next bank draft or make necessary adjustments. You will receive a confirmation email from our Membership & Operations Director, Stephanie Leach stating she has received your request. If you do not receive this email confirmation in 24 hours (Monday-Thursday) please contact Stephanie directly at 262.248.6211 or [email protected] to confirm your change. 

Please note:

  • If you are currently paying the Loyalty Rate, you will forfeit your Loyalty Rate at the time of cancellation.  You can rejoin at any time at the Standard Rate.
  • Membership fees are not refundable or transferable.
  • We understand that accidents happen and medical emergencies can arise.  We offer a medical hold with physician's documentation for a maximum of three-months within a twelve-month period of time.
